This is a custom designed form to fit in a 210 pixel wide sidebar. The background images for each field are custom as is the content inside each of the drop down menus.
The forms you see on this site are minimalist to fit with the theme. But even a minimalist form has custom alignment and border colors to fit the layout and style of the site.
